Terms and Agreements

This is our liability waiver that includes COVID-19 language. All families choosing to participate in Our Little Sunshines  activities must agree to and sign this waiver. 

I understand that it is my responsibility to supervise my child(ren) during participation in Our Little Sunshines soft play and play dates.

I realize that personal contact with others, including but not limited to Our Little Sunshines employees and participants, involves a certain degree of risk of exposure to disease-causing organisms such as COVID-19. I acknowledge that Our Little Sunshines  cannot screen and/or monitor all such individuals. I also recognize that all adults entering the facility, including myself, have agreed to the Our Little Sunshines Illness Policy and have self-screened themselves and their children for COVID-19 at home.

I further understand and agree that I will be financially responsible for all costs incurred to treat any injuries that may occur during these activities, regardless of whether my medical insurance would cover such charges and fees.

I hereby give my consent to my child(ren)’s participation in Our Little Sunshines Activities and hold Our Little Sunshines harmless from any and all liability for any injuries or damages that my child(ren) or I may suffer in connection with Our Little Sunshines Activities. With that said parents are allowed to enter play area to supervise children under 1 year of age. Same rules apply to anyone entering the play area "no shoes, no food/drinks, ect." see rules for futher explination.

Social Media Notice : I understand that Our Little Sunshines may take photos or videos of children and/or adults in the gym. I hereby give Our Little Sunshines, and its affiliates, permission for the copyright and use of these photos or videos on Our Little Sunshines social media websites.
